An eccentric wheel is mounted at the top of the motor shaft; as the motor rotates at high speed, the motion is transmitted to the eccentric wheel via the drive shaft, thereby generating random circular vibrations (similar to a cell phone’s vibration).

When you press the power/on button on the electric toothbrush, components inside the handle move to activate a switch on the circuit board. Once this switch is activated, power is drawn from the battery and transmitted through the circuitry to the motor, which then begins to rotate.
When the motor rotates, the cam and gear system converts the high-speed rotational motion generated by the motor into a back-and-forth brushing motion that cleans the teeth.
